251.055(1)(a) (Supervision Of Children ) on 8/25/2025 : The center self reported that on August 13, 2025, center staff failed to properly supervise 2 children, ages, 2.5 and 3, when both children were left unsupervised for one minute outside of the center's door, while the rest of the class went inside. The children were found by another staff who brought them to the teacher inside. 251.055(1)(f) (Child Tracking Procedure ) on 8/25/2025 : On August 13, 2025, center staff failed to follow the center's child tracking procedure of doing head counts and name to face checks when two children ages 2.5 and 3, where left outside the center's door when the rest of the class went inside.

251.055(1)(a) (Supervision Of Children ) on 2/26/2025 : Based on a self-report: On February 14, 2025, the center failed to supervise a one-year-old child when after returning from the racquet ball court, the child was left outside the classroom for approximately 5 minutes after the rest of the class went inside. The child care workers noticed the child's absence and found the child outside the classroom in the north atrium of the center. 251.055(1)(f) (Child Tracking Procedure ) on 2/26/2025 : Based on a self-report: On February 14, 2025, child care workers failed to implement the center's child tracking procedures of counting children before coming inside and when transitioning to another activity. This failure resulted in a one-year-old child being left unsupervised in the north atrium of the center while the group went inside the classroom.
A warning letter is not an enforcement action under Wisconsin law and carries no appeal rights. Orders and forfeitures are enforcement actions and can be appealed.
