Every day care and residential child care operation HHSC shows on its public Child Care Search (licensed centers, licensed and registered child-care homes, listed family homes, general residential operations, child-placing agencies), with every inspection, investigation and assessment from October 2017 and each cited deficiency with its risk weight and correction dates.

Investigation findings and narratives beyond the cited deficiency, complaint intakes and older history require a Texas Public Information Act request to HHSC.
Corrective and adverse actions are published as current yes/no flags, not dated actions; the date shown is when this site first saw the flag.
The 'sections and standards evaluated' dataset (~1M rows) is not loaded; per-visit counts are deficiencies cited, not standards checked.
Texas Rising Star quality ratings are not in these datasets.
