Known gaps
- Collected from 2026-10 forward from the monthly additions list; older surveys (DHS keeps three years online) are not backfilled yet.
- DQA does not grade assisted-living citations by severity; they are shown as 'severity not stated' and an assisted-living facility is never shown as clean.
How violation severity is mapped
Every cited violation is filed as serious, non-serious or unknown. Unknown is never counted as a clean record.
