Known gaps
- CMS publishes deficiencies as a table (tag and summary), not the inspectors' full narrative.
- Covers the most recent three inspection cycles.
- A few certified nursing homes appear in CMS without a matching HHSC license; they are kept under their CMS number.
How violation severity is mapped
Every cited violation is filed as serious, non-serious or unknown. Unknown is never counted as a clean record.
