- CMS publishes deficiencies as a table (tag and summary), not the inspectors' full narrative (Form 2567). DHS asks that nursing-home statements of deficiencies be requested from its regional offices.

How violation severity is mapped
Every cited violation is filed as serious, non-serious or unknown. Unknown is never counted as a clean record.
