      Observed an air gap is not installed on ice machine where the potential for contamination of the potable water supply exists. Instructed Person In Charge to provide an air gap on ice machine. PIC stated they will get an air gap on ice machine.

      Observed the smoke alarm in rooms 109, 115, 203, 222, 308, and 310 are expired. Could locate expiration on smoke alarms. Did investigations and looked up manufacturer and model of units in each of these rooms. Manufacturer is BRK Electronics and model is 4919 and the other is BRK Electronics model number is 100S. After investigation this models were discontinued between 2004-2007. Also observed yellowing of each unit. Instructed PIC to replace all expired smoke alarm. PIC stated they will replace all expire smoke alarms. PIC stated they will confirm that each bedroom that the alarms are not expire and if they are PIC will replace them.

      Correction: Repair or replace the smoke alarm within 5 calendar days.
