      Observed the 3 compartment sink and sanitizer buckets filled with saniquat is not being used according to EPA registered label use instructions and is less than 150 PPM. Informed Person In Charge. PIC also tested sanitizer and got the same. PIC then discarded all sanitizer and used steramine. PIC then filled all buckets and tested at 400ppm. PIC stated they will resanitizer all food contact surfaces.

      Correction: Provide training to employees on proper use and concentration of sanitizer or repair or adjust warewashing equipment to provide proper sanitizer concentration.

Observed items are missing ingredient list,quantity and name of manufacturer. Provided education to PIC and PIC stated they will label all grab and go food items for students.

      Correction: Label information shall include: common name of food, ingredients in order of predominance, quantity, name of business or manufacturer, color additive such canthaxanthin or astaxanthin in salmon and nutritional information.

      Observed lids for containers used for dry bulk ingredinst are chipping and cracked. Instructed PIC to replace chipping and cracked lids. PIC stated they requested a work order in to replace the lids.

      Correction: Provide equipment that is designed and constructed to be easily cleanable and free from unnecessary ledges, projections and crevices.

Observed no sneeze guard for these food items. Informed Person In Charge that a sneeze guard is required. PIC stated they will make sure self serve foods items that required a barrier will have a sneeze guard.

      Correction: Provide a food shield, food guard, or other effective means to protect food from potential consumer contamination.
