      Observed the private sewage system is not located, designed, constructed or operated in accordance with chapters 382-384 of the Wisconsin Plumbing Code and iss. chapter 145.245. Observed all locks on each holding tank were not lock. Instructed Owner to lock each holding tank lid and maintain it locked. Owner then locked each lock on each holding tank. Observed holding tank on south side on building riser is broken and has a hole on it and vent for holding tank is missing a lid and covered in duck tape. Informed owner of the issue and owner stated they will get it repaired. Infomred owner this will be refered to Planning and Development Department.

      Correction: Referral has been made to DSPS. Provide documentation that the repairs are in compliance with the state plumbing code.
