      Observed closed container of sliced beef made on 3/9/26 is at 48F, closed container of sliced ham at 54F made on 3/9/26 and container of sliced turkey at 46F made on 3/9/26 in 3 door uright cooler have not cooled properly. Informed PIC. Provided education on properly cooling procedures for TCS foods. PIC then discarded containers of roast beef, ham, and turkey.

      Correction: If safe, reheat food to 165°F and restart the cooling process or discard the food item. Food shall be cooled from 135°F to 70°F within 2 hours and from 70°F to 41°F within 4 hours for a total 6 hour cooling period.

      Observed the disheasher using monogram low temp sanitizer is not being used according to EPA registered label use instructions and is at 0 PPM.Tested with Person In Charge and PIC got the same. PIC prime the dishwasher and tested it again and got 0pp. PIC called maintanence to fix it. Dishwasher got fixed and tested it at 100ppm.

      Correction: Provide training to employees on proper use and concentration of sanitizer or repair or adjust warewashing equipment to provide proper sanitizer concentration.

      Observed there is no documentation of an approved variance for reduce oxygen packaging varieties of cold meats. Discussed with PIC and PIC stated they slice the cold meats, vaccum seal then and keep them store them in the freezer up to 2 months. Observed there is no HACCP plan. Instructed PIC to discontinue this process. PIC stated they will remove all ROP cold meats. PIC stated they are interested in getting variance.

      Correction: Submit a variance application to the regulatory authority for submission to the Department for review. If conditionally approved, a copy of the variance shall be kept at the establishment and be subject to review by the inspector upon request. Discontinue current process until a variance can be obtained from the Department.
