     Observed a backflow or back siphonage prevention device is not installed on hose bibs using a hose connected to the wash machine where the potential for contamination of the potable water supply exists. Observed hoses connected to hose bib amd wash machine does not have a backflow preventor device installed. Instructed owner to provide two backflow preventor devices for each hose bib. Owner stated they will get two backflow preventor devices.

     Correction: Install an [ASSE 1011/ASSE 1052]-[threaded hose connection] [ASSE 1055]-[chemical dispenser] compliant backflow or back siphonage prevention device on [describe plumbing item].

     Observed the POWTS access cover was observed unlocked. Observed no locks or chains on POWTS access cover. Instructed owner to contact a service provider to get locks and chains on their access cover. Owner stated they will contact their service provider.

     Correction: Locked or secure the POWTS access cover to prevent general access by the public.

     A vertical separation of less than 2.5 feet measured from the top of the lower mattress to the bottom of the frame of the upper bunk bed was observed be less than 2.5 feet. Measure separation is 25 inches. Spacing needs addition 5 inches. Instructed owner to add additional spacing. owner stated they will get it fixed.

     Correction: Provide a vertical separation of at least 2.5 feet measured from the top of the lower mattress to the bottom of the frame of the upper bunk bed in bedroom on third floor.
