      Employee observed not washing hands after using phone and drinking water. Person In Charge proceeded to rearrange containers of sushi ingredients in prep cooler. Then PIC moved clean knives onto prep table. Intructed PIC to wash their hands. PIC then washed their hands and don a new pair of gloves.

      Correction: Employees shall wash their hands after engaging in any activity that may cause their hands to become contaminated.

      Observed in 2 door sushi prep cooler crab mix at 48F has not cooled within 6 hours. Observed a deep container of crab mix that was covered was prep on 5/19/25. Took side by side temperature and got the same. PIC then discarded container of crab mix. Provided education on cooling.

      Correction: If safe, reheat food to 165°F and restart the cooling process or discard the food item. Food shall be cooled from 135°F to 70°F within 2 hours and from 70°F to 41°F within 4 hours for a total 6 hour cooling period.

PIC stated they had forgot to cut the bags of ROP tuna this morning. PIC stated they will cut the bags open before thawing any ROP fish.

      Correction: Discontiue process involving the variance until corrective actions have been reviewed by the regulatory authority or if a change has occurred, until a new variance has been submitted to the Department for approval. Noncomplaince with variances may result in the variance approval being voided by the Department.
