lots but being the lots will change the layout set by the Planned Development Conditional
Use Permit, we will need to update the Planned Development Conditional Use Permit and
hold a public hearing. Jeremy Skaw of Real Land Surveying also stated that the property
owner would like to change the setback line. Cesafsky drew this on the screen to show

4. Sean Bohan spoke about a project proposal by Jake Nagel of Pebble Creek Construction.
Sean explained that there are multiple steps to this project and that they are at the Plan
Commission meeting to see if there was any opposition to the project from the Plan
Commission. There were questions about access including the roundabout and EMS
access by Misfeldt and Trowbridge. Cesafsky explained that those will all be verified
through the process once the developer secures the land needed. Cesafsky stated there
will need to be an annexation and right-of-way vacation part of the process which will
come back to the City. Mayor Hiess asked the Plan Commission if there was any
opposition to the project and with none, the discussion ended with no action taken.
5. Cesafsky provided a quick background on the request from CF QOZB, LLC and
Steward Wangard for a Planned Development Conditional Use Permit to develop a
186 unit licensed senior living facility with independent living on 13.32 acres south of

Chippewa Crossing. Kristy Schneider of Evolve Architects provided the background on
the layout which includes a 100 unit senior licensed senior living facility, a 58 unit
building for independent living, 6 duplexes, and 4 four-plex’s which will all be managed
by Ebenezer Management Services. Kristy stated the development will provide 40 FTE
positions to the area. Mayor Hiess asked about the cost of the build out. Kristy stated the
estimated build out would cost $47 million. Cesafsky stated that this will need to be a
Planned Development Conditional Use Permit due to the setbacks, height, and
community-based residential facilities being listed as a conditional use under the R3-A
zoning district.
Motion by Misfeldt, seconded by Trowbridge to recommend conducting a public hearing
to consider a Conditional Use Permit resolution for the development of a 186 unit
licensed senior living facility on the south side of Chippewa Crossing. All present voting
aye. MOTION CARRIED.
6. Cesafsky provided the background in the rezone petition by CF QOZB, LLC stating that
the planned development requirements will have the underlying zoning district of R3-A
and therefore the rezoning of the 13.32 acre parcel will need to be R3-A.
Motion by Tzanakis, seconded by Varga to recommend Common Council approve the
rezone petition and hold a public hearing for the request of CF QOZB, LLC and
Stewart Wangard to rezone the 13.32 acre parcel from C-2 General Commercial District
to R3-A Multiple Family Residence District. All present voting aye. MOTION
CARRIED.
7. Cesafsky provided information that the 2 lot Certified Survey Map on the south side of
Chippewa Crossing for CF QOZB, LLC and Stewart Wangard has been reviewed by the

of Lafayette. There was a question about why the City sees these Plats to which the
Mayor explained that the City gets the privilege to review extraterritorial plats and review
them based on our design standards in case we were to ever annex them into the City in
the future.
Motion by Misfeldt, seconded by Trowbridge to recommend common council approve
the Wissota Lane Extraterritorial Plat in the Town of Lafayette. All present voting aye.

CITY OF CHIPPEWA FALLS PLAN COMMISSION RESIDENTIAL PLANNED
DEVELOPMENT CONDITIONAL USE PERMIT RESOLUTION NO. 2025-05
TO AMEND CUP RESOLUTIONS #07-03, #12-03, #12-04, #14-01, AND #18-02
FOR PHASE V DEVELOPMENT OF WILLOW CREEK SUBDIVISION
WHEREAS, on May 7, 2007, the Plan Commission conducted a public hearing and approved
CUP Resolution #07-03 (available for inspection in the City of Chippewa Falls Engineering
Department) to Heartland Contractors, Inc. for a Planned Unit Development; and
WHEREAS, on May 7, 2012, the City of Chippewa Falls Plan Commission approved CUP
Resolution #12-03 allowing temporary row crops on phases 5-7 of the development, (approximately 46
acres); and
WHEREAS, on June 11, 2012, the City of Chippewa Falls Plan Commission approved CUP
Resolution #12-04 allowing changes in housing type in Phase II of the Willow Creek Subdivision plan.
The approved changes were replacing 10 Single Family lots with 12 Twin Home lots and replacing 28
Single Family lots with 15 Multi-Family complexes lots;
WHEREAS, on April 7, 2014 the City of Chippewa Falls Plan Commission approved PD CUP
Resolution #14-01 reducing 13 Single Family lots located around Brooke Court to 9 larger Single
Family lots in Phase I and changing 22 Single Family lots to 30 Twin Home lots adjacent to Timber
Trail in Phase III. The proposed net change for the Willow Creek Subdivision was a four-unit increase
resulting in 536 units in 190 acres or a 2.8 unit/acre project density.
WHEREAS, at its’ April 9, 2018 meeting, the City of Chippewa Falls Plan Commission heard a
request from Bill Albright, representing Heartland Contractors, Inc., for a Planned Development
Conditional Use Permit for Willow Creek Phase IV to allow a reduction of 3 Single Family lots, an
addition of 6 Two-Family (12 units) lots and a reduction of 16 Single Family Bay Home lots. This
results in a Phase IV project density reduction from 5.8 units/acre to 4.95 units/acre; and
WHEREAS, the Plan Commission was presented with a revised Phase IV plat, General
Development plan, Single-Family and Two-Family Home Development Standards, Typical Photos,
Elevations, and Floor Plans, and existing and revised Phase IV Land Use and Zoning site plans; and
Planned Development Conditional Use Permit Resolution No. 2025-05
Page 2 of 4
WHEREAS, the Plan Commission evaluated the development proposal in accordance with
Municipal Code Section 17.26(16)(d)1 which directs such evaluation as a Conditional Use Permit under
Section 17.47; and
WHEREAS, on May 7, 2018 the City of Chippewa Falls Plan Commission approved PD CUP
Resolution #18-02 after publication and mailing of all required notices and hearing all concerns and
comments; and
WHEREAS, on March 10, 2025 the City of Chippewa Falls Plan Commission voted to conduct
a public hearing to hear all concerns about the amendment of the Planned Development Conditional Use
Permit Resolution #18-02 on April 7, 2025; and
WHEREAS, on April 7, 2025 the City of Chippewa Falls Plan Commission conducted a public
hearing to hear all concerns and approved PD CUP Resolution #2025-05 after publication and mailing
of all required notices and hearing all concerns and comments; and
NOW, THEREFORE, BE IT RESOLVED, that the Plan Commission of the City of
Chippewa Falls finds;
1. That upon review of PD CUP Resolution #2025-05 and the approved Storm Water Management
Plan by City Staff, the proposed reduction of 3 Two-Family lots, an addition of 2 Single Family lots will
result in the similar common space and green space as the original Phase V layout and will result in
decreased density.
2. The Plan Commission further finds that the revised Phase V plat and land use plan are not
detrimental to adjacent land uses because the revisions do not present any substantial changes in land
use intensity.
3. The Plan Commission further finds that the revised plat and land uses are consistent with the
original purpose and intent of CUP Resolutions #07-03 and amendments #12-03, #12-04, #14-01, and
#18-02 because the revised lot sizes, areas, widths and setbacks represent a similar intensity of land use.

Living, and Memory Care will be served by an 8” sanitary service and a 6” water service. Private
sewer and water mains throughout the site will be installed to serve the development.
Connections to the public utility will be done along the north side of the project site.
PD CUP Density:
The Duplexes will have a total of 12 units. The Four-plexes will have a total of 16 units. The
Active Living Center will have a total of 58 units. The Assisted Living Center will have a total of
100 units. The site will have a total unit count of 186 units. The site is 13.32 acres which will
create a site density of 13.96 units per acre.
Civil Engineering Design:
The civil engineering design for the project will follow the City of Chippewa Falls and the State of
Wisconsin design criteria. All required permits will be obtained through the state and city.
Design plans incorporating proper drainage, street design, street signage, and street lighting will
be submitted to the City of Chippewa Falls for approval. Note that the roads & utilities
throughout this site will be private.
Impact on local workforce development and employment opportunities:
The project will employee about 40 FTEs once it reaches stabilization, which is projected for the
end of the second year of being open. Employment opportunities will include resident
assistants; registered nurses; other care staff, such as activities and Dimensions manager
(Ebenezer’s memory care program); dietary staff; housekeeping; maintenance; and
administrative positions.
The impact of this senior living development to Chippewa Falls and specifically to Chippewa
Crossing is nice balance to the recent restaurants, grocery, gas/convenience store, medical
office, and proposed new multi-family housing. Finding and promoting that balance to the city
growth helps to enhance the quality of life for all residents and enhances the architectural
character of Chippewa Falls along with enhancing the values of the property through finding the
highest and best use for the land. This ties directly into the city planning efforts to encourage a
mix of new housing options. This project is one component of a mix that includes medium
density rental housing for workforces, seniors, and students that will contribute to supporting
retail and business in the city.
Economic benefits of this development:
This senior living project will bring a much bigger impact to Chippewa Falls than just another
apartment building. Some facts to note:
• Employment economic impact: This senior living project will employ approximately 40
FTEs, with about $2.0 million in wages annually once the project stabilizes. With the
nursing and CNA program at CVTC and the university nearby, the project will provide
good jobs here in the community for a range of careers.
• Economic impact to local businesses: The project plans for a mix of active 55+ senior
living units and the independent living residents and are likely very similar to the
residents that the city they will likely be a mix of singles and couples who are active and
out walking and driving, going to the grocery store, restaurants, going to concerts, going
to church, etc.
Each independent and assisted living unit has its own kitchen where residents can
choose to prepare their own meals. Traditionally, about 50% of independent living
residents prepare the majority of their own meals. The assisted living and memory care
residents may not be as active in the community, but certainly they will be utilizing local
healthcare and other services, as well as drawing family to the community to visit their
loved ones.
For the big picture -- all the seniors living here will create a significant economic impact
through their spending throughout the community. The local medical clinics, specialty
medical services, and the local hospital will benefit by serving the seniors within this new
Chippewa Falls senior project. Local pharmacies, local insurance, local banks will benefit
from proving local services. Local restaurants and hotels will benefit from family and
visitors coming to the new facility. UWEC and Chippewa Valley Technical College will
see a benefit in collaborating with the facility for opportunities for their students and
graduates in nursing, physical therapy, social services, and other medical careers. Like
the apartment building would have, the senior living community will contract with local
service providers for lawn maintenance, snow blowing, waste removal, utilities,
insurance, etc.
• Economic impact of City of Chippewa Falls' seniors selling their homes and
staying in the community: When seniors move from their homes to a senior living
community, most of them are homeowners. When they sell their homes, that opens new
housing opportunities for other people/families to purchase homes in the community.
This new facility will provide capacity here in Chippewa Falls for current residents to be
able to continue to live in our community and not have to move elsewhere when they are
ready to move out of their homes.
• Economic impact of new seniors moving to Chippewa Falls: The market demand
assessment for this project which helped us determine the project size and unit mix
shows in the Primary Market Area (PMA) that 70% of residents to come from with this
PMA, and 30% will come from outside the PMA. In any case, many new residents will be
moving to the city of Chippewa Falls to live here.
• Economic Impact of Residents’ Families, Corporate Staff and other Vendors: Each
person would have, on average, five different visitors a week. Some visitors will be
Chippewa Falls residents, and others will be coming from surrounding communities.
Visitors often like to take residents out and this site enables them to take them out for
lunch at local establishments with ease. Outside vendors and corporate staff visiting the
staff also frequently stop for lunch when they are in town. All of these people would be
likely stop to pick up something for their way home or check out specialty shops when
they are in the area.
• Construction economic impact: The senior living project will take about 20 months to
build. On a similar-sized projects we had between 40-60 people on site every day for the
duration of the project. The addition of these 40-60 construction workers in the city for 20
months will certainly make an economic impact as well.
• Serving Low Income Senior Residents – As the project opens, the model is planning
for four of the thirty-two memory care units to be rented to low-income residents that are
eligible to receive assistance from the state through Wisconsin Community Options
Program Waiver (Family Care). The model is also planning for three Elderly Waiver
assisted living residents starting in the third year of operation. The idea here is that as
assisted living residents spend down their assets, we want them to be able to stay in the
place that will become their home. We want to serve the broader needs of our
community.
Anticipated timeline:
Depending on city approvals, we anticipate the construction to start in Fall 2026 and be ready
for move in by late spring 2028.
Other noteworthy items:
The building designs and development site layout are thoughtfully designed with our past work
with several senior living projects in the last 20 years. The following are just a few of the
additional highlights:
• The project focuses on reducing the size of the surface parking lot area and designed
the underground parking for approximately 60% of the parking spaces. This leaves
more room for landscaping, courtyards, an outdoor pavilion, games, walks, gardens, etc.
• Site parking is spread out and located inward on the site layout so the larger buildings
are the focus from Chippewa Crossing.
• Extensive landscaping and open space with walking paths, benches and sitting areas,
dog walking, pavilions and gardens.
• The parking areas will have walking paths for pedestrians.
• The architectural style, materials, and treatments will be reviewed to help enhances the
character of Chippewa Falls.
• Mechanical equipment will be located on the rooftop areas to be out of view and the
trash collection is located in the underground parking level and out of view in enclosures.
• The service delivery drives are located on the south side of the building entrance and
visibly away from Chippewa Crossing and designed to be a part of the site circulation
and parking.
Summary:
The site will have various options for senior residents including independent living or assisted
living spaces. There will be a total of 186 units of various types provided on the site. There will
be two access points onto the site. All buildings will be served by private services. The site will
utilize the regional storm pond to the south for any stormwater runoff.
