County Health, Dental and Vision Utilization for 202 6 and Possible Plan Adjustments for 202 7 –
Presentation by USI
Alex Beaudry – USI presented a PowerPoint outlining the County’s 2027 pre -renewal strategic plan which
included: Historical solutions in review and 2026 renewal summary; Experience Report June 2026 including net
total of all claims, net plan costs, total F IE (fully insured equivalent), net claims, large claimants; 2026 FIE vs.
actual cost; 2025 FIE vs. actual cost; 6 year action vs. trend; Mid-year renewal projection – projecting a 14.11%
increase to rates, Stop Loss Carrier has contacted the County for ear ly renewal – this is a good indication that
the renewal may be less than anticipated, should the renewal increase come in less than 5% USI would like pre-
approval to renew earl with our current stop loss carrier; review of when a decision can be made regar ding
premiums / how long can the County wait to determine premiums, open enrollment timing; Garner Executive
Summary – 80% sign up, 94% member satisfaction, 3.8 specialties recommended per engaged member, $269K
estimated savings; Garner Renewal Su mmary – recommendation: 3-year renewal at 100% settlement return;
Family Savings Plan - Avoided claims report, shared savings calculation; Plan Design Alternatives – plan design
compared to public sector employers, and state; Employee Payroll Contributions by single plan and family plan;
Plan Design Alternatives – 1. Reduce Garner incentive from $2,000/$4,000 to $1,500/$3,000 (savings of 3% or
$240,000), 2. Increase coinsurance from 10% to 15% (savings of 1.5% or $120,000), 3. Increase deductible from
$2,500/$5,000 to $3,500/$7,000 (no changes to current Garner reimbursement) (savings of 7% or $560,000), 4.
Change ER copay to copay, then deductible, then coinsurance (savings 0.5% or $40,000), 5. Change Wellness
Incentive – increase premium contributions.
Recommendations:
• Wait as long as we can, health plan is trending in a positive direction and a month or two of additional
data should help lower the renewal
• Stop Loss – renew if less than 5%
• Garner – renew for 2027
• Family Savings Plan – continue for 2027
• Dental – no change
• Vision – no change
• Plan Design Changes
o If overall renewal is below 10% no recommended changes
o If above 10% look to deductible changes or changing the Garner HRA program
Discussion followed and included: continue to work with Door County Medical Center; timing of next meeting to
make final decisions.
Stop Loss Renewal
Motion by Shaw, seconded by Jeanquart to approve moving forward with the Stop Loss renewal if the anticipated
increase is less than 5%. Motion carried by voice vote.
2027 Projected Wage Aging & Pay for Performance
Cunningham Butler is the County’s wage consultant. Consultants recommend at 3 – 4% wage increase for 2027;
current budget includes a 3.5% increase (2% aging of plan on 1/1, plus 1.5% wage increase on employees
performance review at the end of March). Employees below 100% would receive the increase; those at or above
100% would receive a one -time bonus. Can adjust either the aging or the increase. Approximately 40% of
employees are at 100% or more. Aging the plan ke eps the pay plan current in the market. Discussion followed
and included: when was the last compensation study completed and implemented (2023/2024); could a longevity
bonus be considered ; what are other counties doing with pay ; are benefits still a compet ing factor to attract
employees; provide other options to increase wages such as split year increases.
Increase of Hours, New Positions, Reclassification Requests, and Part Time Pay Rates for 2027

Public Health Nurse
(Market Review) 3
The County has experienced significant difficulty recruiting and retaining qualified Public
Health Nurses since the COVID-19 pandemic. Vacancies often remain open for extended
periods, applicant pools are limited, and qualified candidates frequently accept
employment with hospitals, health systems, or other healthcare organizations that offer
higher starting wages and greater earning potential.
Recommended that there be no change.
Jan-27 Yes
Public Health Nurse
(Reclassication) 1
Change to Public Health Nurse Supervisor. In 2022, the County hired its first Public Health
Officer without a nursing background. This shifted the supervision of all direct patient care
to a staff member. The position took lead roles in training and oversight of new staff
regarding work specific to the hands-on nursing work done in the division. This includes
contact tracing, immunizations, coordinating vaccination clinics, prenatal care, and care
coordination.
Recommended to place at Pay Grade J.
Jan-27 $136,474.00 $144,315.00 $7,841.00 $7,841.00 Yes
Birth to Three Educator and Service Coordinator
(Reclassication) 1
There has been an increase in duties over the past 14 years, primarily related to the
changes at the state level in terms of increased requirements.

Foster Care Coordinator
(Reclassication) 1
The role is being moved to a Social Worker position. The actual duties of the role have been
more of the responsibilities of a Social Worker. The Foster Care Coordinator position would
be eliminated.

DTE Peer Recovery Support Specialist
(New Position) 1
Create Peer Recovery Support Specialist (PRSS) position. This role is new in the treatment
array of services. We do not currently have a PRSS position in our organizational chart. In
June, that service became a billable service for those individuals who have BadgerCare. It is
proposed that the position be developed as a Defined Term Employee (DTE). It would give
us a six-year window to use this service and evaluate the effectiveness of the service. There
would be no tax levy effect.

ADRC
ADRC Director
(Reclassication) 1
At the end of 2025, Adult Protective Services (APS) was transitioned under the supervision
of the ADRC Director. While this change resulted in the addition of three staff members to
the position’s supervisory responsibilities, the more significant change is the nature and
complexity of the services this role is now responsible for.
Recommended to change to Pay Grade L.

ADRC Specialist
(Reclassication) 3
The ADRC Specialist position is currently classified at Pay Grade F, below both the Disability
Benefit Specialist and Elder Benefit Specialist positions, which are classified at Pay Grade G.
These positions have comparable educational and experience requirements, with all three
requiring a bachelor’s degree and relevant professional experience. The ADRC Specialist
position serves as a primary point of entry for individuals seeking long-term care, aging,
disability, and community-based resources, which requires knowledge of multiple service
systems, programs, and funding sources. As a result, the work is equally, if not more,
complex than these comparable positions.
