The child was found to be missing once the class was outside approximately 2-2.5 minutes later during a name-to-face count. 251.055(1)(a) (Supervision Of Children ) on 4/10/2026 : The center self reported that on March 19, 2026, a child care worker failed to supervise a four-year-old child when escorting a separate class to another classroom. The worker told the child to go to the child's room, but did not observe the child going into the classroom.
