251.055(1)(a) (Supervision Of Children ) on 4/14/2026 : The center self-reported that on March 31, 2026, child care workers failed to supervise 2.5-year-old child and a 3-year-old child when they left the classroom without staff knowledge. They were observed in a hallway by the cook running towards an exit door. While the cook went to get help, the children were out of sight and sound supervision for approximately 2 minutes. 251.055(1)(a) (Supervision Of Children ) on 4/24/2026 : The center self-reported that child care workers failed to supervise children when a four-year-old child reported that another four-year-old child touched him inappropriately in the calm down corner. Staff are unable to provide sight and sound supervision to the class when they are assisting children in the bathroom. 251.055(1)(f) (Child Tracking Procedure ) on 5/12/2026 : On May 8, 2026, a child care worker failed to adhere to the center's tracking procedure of a name-to-face count when she left a three-year-old child outside while the rest of the class went inside. The worker was not aware that the child was missing and had the child checked as present on the tracking form.
