Dear Records Officer,

Under the Minnesota Government Data Practices Act, Minn. Stat. ch. 13, I request copies of the following child care licensing records for Active Child Care Center, Inc, 2000 Plymouth Ave N, Minneapolis, MN 55411-3713 (license 1093196), for January 1, 2019 to the present:

1. Every licensing inspection or monitoring visit report, with the rule violations cited.
2. Every correction order, conditional license, fine, suspension, revocation or other licensing action, with related correspondence.
3. Every complaint and maltreatment or investigation determination, and its outcome.
4. Every correction plan the program submitted, and the agency's response.
