     Attendance was not current and accurate when on the 7/14 attendance, multiple children were not signed in to any classroom. A 1.5-year-old child was signed in to the opening room from 5:45-6:06 am and was next signed in to the Toddler room at 7:55 am. A 2-year-old child was signed in to the opening room from 5:03-6:06 am and was next signed in to the Two's room at 7:10 am. A 3-year-old child was signed in to the opening room from 5:58-6:06 am and was next signed in to the Preschool room at 7:10 am.

     According to staff, on multiple occasions, unqualified assistant teachers have been left alone and unsupervised with groups of children outside of the opening and closing hours.

     On 7/14, the center did not meet minimum ratio requirements when in the: Two's room, one teacher was with 9 children for 13 minutes when two teachers were required. Preschool room, one teacher was with 14 children for 25 and 46 minutes when two teachers were required. Pre-K room, one teacher was with 17 children for 1 hour and 41 minutes when two teachers were required School-Age room, one teacher was with 20 children for 1 hour and 35 minutes when two teachers were required. According to staff interviewed, there were multiple classrooms that were out of ratio on this date.

     Staff A used prohibited actions that may be psychologically, emotionally or physically painful, discomforting, dangerous or potentially injurious on two different occasions. During the licensing visit, they grabbed a school-age child's arm to move the child away from the table and the child yelled, "Ow." Additionally, according to a complaint, Staff A was seen aggressively ripping a cup out of a child's hand and yelling at the child.

     According to staff, children are not allowed to get up off their cots after 30 minutes of rest, during a 2 to 2.5 hour nap time. Children are allowed to take items back to their cot, but are not allowed to get up from their cot as required. According to staff, this practice is used throughout the center.
