While in the gym, a child under the age of 7 informs Staff B they need to use the bathroom. Staff B instructs a child over the age of 7 to leave the gym to take the younger child to the bathroom which is located outside of the gym. While they are gone, Staff B remains in the gym with the rest of the children. 251.06(2)(i) (Deteriorating Paint ) on 4/3/2025 : Several areas of deteriorating, flaking and cracked paint observed on the walls are accessible to children in the cafeteria. 251.094(5)(a) (School-Age Group Leader - Supervision ) on 4/3/2025 : Staff B is not working under the supervision of a school-age program leader during the monitoring visit. 251.095(2m)(c) (School-Age Care - Supervision Of Group Of Children ) on 4/3/2025 : At least one school-age program leader or child care teacher is not supervising each group of school-age children during the monitoring visit when Staff B is the sole staff member working when a group of 13 school-age children are in care. Staff B meets the qualifications of a school-age group leader.

Programming is not suitable for the developmental level of each child. Over the course of the 90+ minute visit children were only provided with approximately 10 minutes of programming. The remaining program time is spent with children seated at a table with no activities and then repeatedly going up and down three floors in the building either by stairs or elevator to gather children's belongings, waiting in the hallway while children are sent to other floors to gather other's belongings, and walking throughout the building to release children to parents who are outside the school.

The program failed to provide daily periods when a variety of experiences are concurrently available for the children to select their own activities. A group of children were required to sit at a table following snack during which time they were not permitted to leave the table and were instructed to speak about any upcoming weekend plans. When one child attempted to utilize art equipment they were instructed to return to the table. While in the gym children are observed seated on the floor with no activities when the only available activity is a group game or small bean bags.
