  251.055(1)(a) (Supervision Of Children ) on 6/13/2025 : On 05/28/2025, Child 1 was without site and sound supervision when they exited the classroom. On 06/03/2025, Child 1 was without site and sound supervision when they were left in the outdoor play space unattended. Both incidents were self-reported by the center. 251.055(1)(f) (Child Tracking Procedure ) on 6/13/2025 : On 05/28/2025, Child 1 exited the classroom without staff knowledge. Staff did not implement the center's child tracking procedure to ensure all staff know children's whereabouts at all times. On 06/03/2025, the center's child tracking procedure failed when Child 1 was left in the outdoor play space while the rest of the group transitioned inside. Both incidents were self-reported by the center.
