      The premises are not clean and in good repair when the entry door threshold is separating from the bottom sill, a shed attached to the center has loose shingles that are coming off, and there is a large jagged hole in the siding in the children's outdoor play space at child level. There is an open crack in the wall at child level in the Dragonfly Room, the basement floor between classrooms is dirty with crumbs and debris, and nearly the entire interior surface of the Butterfly Room exit door is covered in dirt.

      The center's written plan for a tornado emergency states staff lead children to designated safe area in the basement; during the licensing visit, staff identified the basement storage closet as the tornado shelter and pointed it out. When the basement storage closet was opened, there were several items blocking entrance into the room.
