251.055(1)(a) (Supervision Of Children ) on 5/4/2026 : On April 28, 2026, two staff members failed to properly supervise a 4.5 year old child when the child was left alone in the center's playground for about 3 minutes. 251.055(1)(f) (Child Tracking Procedure ) on 5/4/2026 : On April 28, 2026, two staff members failed to follow proper child tracking procedures in the playground when a 4.5 -year-old child was left unsupervised in the playground for approximately 3 minutes after his group went inside the building.

251.055(1)(a) (Supervision Of Children ) on 9/15/2025 : Based upon self report on September 10, 2025, a child care worker failed to supervise a four-year-old child when the child went to the bathroom while their class transitioned to the outdoor play space without the worker's knowledge. The child found a child care worker who brought the child outside to the group. 251.055(1)(f) (Child Tracking Procedure ) on 9/15/2025 : Based upon self report, on September 10, 2025, the child care worker did not conduct the name to face count of children in care at all the transitional points while taking the children from the classroom to the outdoor play space resulting in a four-year-old child being left behind.

A warning letter is not an enforcement action under Wisconsin law and carries no appeal rights. Orders and forfeitures are enforcement actions and can be appealed.

Under Wisconsin's public records law, Wis. Stat. §§ 19.31–19.39, I request copies of the following records for the child care program Ozaukee Child Care And Preschool, 1195 County C, Grafton, WI 53024-9727 (DCF facility number 1013651; provider number 0000558680, location 006; applicant/licensee number 8100002886), for January 1, 2019 to the present:

1. Every licensing or certification monitoring, renewal and follow-up visit report, including noncompliance statements, compliance statements and rule-monitoring summaries.
2. Every complaint investigation: intake summaries, investigation reports, and the finding for each allegation.
3. Every warning letter, order, forfeiture or other enforcement action, with related correspondence.
4. Every plan of correction the program submitted, and the department's response.
5. Every incident the program reported to the department (for example under DCF 251.04(3) or 250.04(3)): injuries requiring medical care, prohibited actions, suspected abuse or neglect, communicable disease, and children whose whereabouts were unknown.
