251.055(1)(a) (Supervision Of Children ) on 2/6/2026 : The center self reported that on February 2, 2026, while the group of children were coming inside a two-year-old child left their group and went into an open classroom. The child was out of supervision for approximately one minute. 251.07(3)(a)1. (Indoor Equipment - Developmental Level, Size ) on 2/6/2026 : The center self-reported a child requiring medical attention. On January 26, 2026, a 1.5-year-old child was playing with magnetic tiles rated for ages three years and up. The child swallowed magnets inside of the tiles. 251.07(3)(a)2. (Indoor Equipment - Construction, Condition ) on 2/6/2026 : The center self-reported a child requiring medical attention. On January 26, 2026, a 1.5-year-old child was playing with magnets that were not in good condition. The child was able to pull apart the cracked magnetic toy. The child obtained two small magnets and swallowed them, requiring medical attention.

251.055(1)(a) (Supervision Of Children ) on 1/20/2026 : The facility self-reported that on January 13, 2026, child care workers failed to supervise a two-year-old child when the class was in the hallway removing their outdoor gear, for approximately one minute, when the child wandered down the hall and was found by the front office staff member who returned the child to the classroom.

A warning letter is not an enforcement action under Wisconsin law and carries no appeal rights. Orders and forfeitures are enforcement actions and can be appealed.

Under Wisconsin's public records law, Wis. Stat. §§ 19.31–19.39, I request copies of the following records for the child care program Davis Child Care Center, 1260 N Westfield St, Oshkosh, WI 54902 (DCF facility number 420377; provider number 0000559770, location 003; applicant/licensee number 2100003828), for January 1, 2019 to the present:

1. Every licensing or certification monitoring, renewal and follow-up visit report, including noncompliance statements, compliance statements and rule-monitoring summaries.
2. Every complaint investigation: intake summaries, investigation reports, and the finding for each allegation.
3. Every warning letter, order, forfeiture or other enforcement action, with related correspondence.
4. Every plan of correction the program submitted, and the department's response.
5. Every incident the program reported to the department (for example under DCF 251.04(3) or 250.04(3)): injuries requiring medical care, prohibited actions, suspected abuse or neglect, communicable disease, and children whose whereabouts were unknown.
