  251.055(1)(a) (Supervision Of Children ) on 9/19/2025 : On 9/19/25, child care workers left their assigned room to grab extra food from the kitchen leaving the children in the infant/toddler room unsupervised. That staff member did say there was no help for preparing food so this is common practice. 251.09(1)(L) (Infant & Toddler - Soft Materials In Cribs ) on 9/19/2025 : On 9/19/25, upon observation there was a blanket in the pack and play of a child under 1 year old. There was a second child asleep in the swing with a light blanket wrapped around them. 251.09(2)(bm) (Infant & Toddler - Sleep Position ) on 9/19/2025 : On 9/19/25, there were 2 infants observed to be sleeping in swings. Staff stated that this is used for the short naps around lunchtime for the older children and typically is 15-20 minutes in length.

A warning letter is not an enforcement action under Wisconsin law and carries no appeal rights. Orders and forfeitures are enforcement actions and can be appealed.

     Based on observation, on August 11, 2026, a staff member walked out of the preschool classroom several times to retrieve the children's cots from the hallway, leaving 6 children alone in the classroom. The same staff member also allowed a 5 year old child go to the bathroom alone, while trying to supervise the classroom, however, there was a wall in the hallway preventing the staff from properly supervising both.
