251.055(1)(a) (Supervision Of Children ) on 12/16/2024 : On 12/12/24, two children (age two and a half and three-years-old) were in an empty classroom, without sight And sound supervision, for approximately two minutes. 251.055(1)(f) (Child Tracking Procedure ) on 12/16/2024 : While classmates and teachers were washing hands in a bathroom, two children left the remainder of the group and went to an unattended classroom. Staff were unaware of their whereabouts until the children were returned to the group by the administrator.

  During a licensing visit, I observed three children under the age of 7 years, walk down a hall and into a bathroom alone. They did not have sight and sound supervision by a caretaker.

  This licensing specialist observed a gap in an exterior door at the back of the building. The opening was 1/2 inch in height between the doorframe and the bottom of the door, and 4 inches in width. An opening of this type would allow cold air into the building and possibly, the entrance of very small vermin.

Under Wisconsin's public records law, Wis. Stat. §§ 19.31–19.39, I request copies of the following records for the child care program Children'S House Montessori Sch, 415 E Lake St, Eau Claire, WI 54701 (DCF facility number 1012359; provider number 0000564650, location 002; applicant/licensee number 3100008537), for January 1, 2019 to the present:

1. Every licensing or certification monitoring, renewal and follow-up visit report, including noncompliance statements, compliance statements and rule-monitoring summaries.
2. Every complaint investigation: intake summaries, investigation reports, and the finding for each allegation.
3. Every warning letter, order, forfeiture or other enforcement action, with related correspondence.
4. Every plan of correction the program submitted, and the department's response.
5. Every incident the program reported to the department (for example under DCF 251.04(3) or 250.04(3)): injuries requiring medical care, prohibited actions, suspected abuse or neglect, communicable disease, and children whose whereabouts were unknown.
