Our intention is to have in-person meetings going forward. For the time being, we will hold the City Committee Meetings, Plan Commission, Council and most others at the Community Room at 933 Michigan Avenue. DPC can provide easier, more convenient access to primary care while potentially reducing overall healthcare costs. City Of Stevens Point 1515 Strongs Avenue Stevens Point, WI 54481-3594 Page 3 of 7 September 9, 2026 MEMORANDUM TO: Personnel Committee Members FROM: Sandy Frasch, HR Manager RE: Consideration of 2027 Health Plan Renewal Recommendations and Benefit Options The City’s self-insured health plan had a 95% loss ratio in 2025, with no premium increase for 2026. The 2026 year-to-date loss ratio has increased to 114%, meaning claims are exceeding premiums collected. As a result, plan changes are being considered for 2027 to help manage costs and minimize premium increases. The initial projected rate increase was 23%. Adoption of the recommended OptumRx Premium PLD formulary and Pass-Through pricing model is expected to reduce the increase. While the prescription changes affect approximately 2.2% of prescriptions, they are projected to generate approximately 30% in prescription savings and improve the plan’s funding forecast. Ethica recommends implementation effective January 1, 2027. Supporting documents are attached, including 2027 Pharmacy Renewal Recommendation, and OptumRx Reprice Summary. Based on our analysis, Ethica recommends: • Move from Select Comprehensive Formulary to Premium PDL Formulary • Elect a Pass-Through Pricing Contract This combination provides the greatest overall value through lower net pharmacy costs, improved rebate opportunities, and minimal member disruption.
