Community-Based Mental Health and Substance Use Psychosocial Rehabilitation Services for Children and Adults For Emergency Services Call: During office Hours (715) 345-5350 After Hours/Weekends/Holidays (866) 317-9362 Portage County Health and Human Services 817 Whiting Ave. Stevens Point, WI 54481 Portage County provides employment and services to any eligible person without regard to age, race, religion, color, sex, national origin, ancestry, handicap, physical condition, developmental disability [as defined in s.51.01 (5)], arrest or conviction record [s.111.32], sexual orientation, marital status, or ability to pay. Questions about CCS…. • Who is eligible for CCS? CCS is available to adults and children who are Portage County residents, eligible for Medicaid and are determined to need psychosocial rehabilitation services for a mental health or substance use diagnosis. • How do I apply for CCS? Please contact the agency at (715) 345-5350 and push option 2 for adults needing CCS Services or option 5 for children needing CCS service. Health and Human Services will determine the applicant’s eligibility and need for psychosocial rehabilitation services through a screening process. • How do I pay for CCS? CCS is a Medicaid program. Participants must be eligible for Medical Assistance, Badgercare or other Medicaid programs. Page 3 of 8 Service Planning… A “Service Plan” is a written plan of services to be provide or arranged for that is based on the comprehensive assessment. A Service Plan will be developed with the Recovery Team that addresses the needs and goals identified in the assessment process. Consumer Rights…. All rights in the Your Rights and the Grievance Procedure brochure apply to CCS. In addition CCS consumers have the right to: • Choose the members of your Recovery Team. • Receive specific, complete, and accurate information about proposed services. • Consent to treatment and to withdraw from the CCS program at any time. Rights will be explained during enrollment into CCS. CCS Advisory Committee…. An advisory committee consisting of 1/3 CCS consumer membership provides input to CCS on services, policies and consumer rights. Comprehensive Community Services... CCS is a voluntary community- based program for children and adults diagnosed with a mental health and/or substance use condition. CCS offers psychological, social, and substance use rehabilitation services. These services and supportive activities assist consumers with learning the skills necessary to achieve their highest level of independent functioning, stability, and independence to facilitate recovery. A primary goal of CCS is to provide services through a single coordinated system of care and support consumers to participate as equal partners. A fully integrated system of care ensures services are based on a shared set of values and work together to support the consumer in reaching their goals. Recovery Team… When an individual is admitted to CCS, a service facilitator (case manager) and mental health professional are designated to work with them. A Recovery Team is then formed that includes the consumer, service facilitator, mental health professional, service providers, and any other individual the consumer wishes to include for advocacy and support. If a consumer already has a case manager at PCHHS that person may also serve as a CCS team member. Assessment… An “Assessment” is the process used to identify the strengths, needs, and desired outcomes of a consumer and to evaluate progress towards desired outcomes. The first step in understanding the strengths and needs of a consumer is to conduct a comprehensive assessment in partnership with the Recovery Team. During the assessment process the consumer will be asked to provide information about their background, concerns, desires, and goals. Confidentiality… CCS services are confidential mental health services. Consumer informed consent is required to share information about consumer services with outside individuals or organizations. Discussion/Possible Action Re: Approval of 2027 New Position Request for an Economic Support Specialist (1.0 FTE) A motion was made by Amber Garner, seconded by Landon Elgersma, to Approve the 2027 new position request for an Economic Support Specialist.
