An unannounced follow up inspection was completed on 8/5/25. Staff reported the children are not left alone, and there are 2 staff present at all times. They stated they have 1 night staff present at nighttime and this was recent increased to 2 staff. They reported they review supervision based on a list of children and the level of supervision needed. It was reported they completed a suicide risk assessment training on 7/31/25. However, staff interviewed did was not able to confirm they attended a training. Children were interviewed during inspection and reported not having any protocol or people talk to them about suicide or high-risk behaviors in case they exhibit suicidal ideation warnings signs. They did report having a therapist that provides therapy virtually. Staff reported they don't complete assessments, but they follow protocol by calling their higher ups when a child shows high risk/Suicide risk behaviors. Owner reported they complete assessments once they take the children to the hospital for assessment, and/or have the Treatment Director come to the operation and assess the children. If it is determined the child will not be hospitalized, they reported to increase close supervision. However, a paper/computer screening was not reported to be completed.
