During the inspection walk-through, the cabinet that the psychotropic medication (was not double locked), all other medications, knives, and multiple other objects was found defective as the handle was locked, but the kick plate was not holding the door shut. The handle itself was locked, but the door was able to be pulled open. The van keys were also stored in this closet, allowing access to the child, when they overheard the door was broken. Three staff were aware of the broken door prior to the incident.
